Application for Admission

The following document contains all forms students may provide in applying to St. Mary High School. All documents must be completed by December 31st for review.

Mandatory Documents

• The Application for Admission must be completed by all students

Additional/Optional Documents

• Scholarship Application may be completed by any student
• Transcript Upload may be completed for any student
• Teacher Recommendations can be mailed, emailed, or completed using the online form which can be found under the Admissions dropdown menu

Financial Aid Application

• To apply for Financial Aid, visit the Tuition Assistance page of our website (Prospective Students tab) and complete the FACTS Application Online
• All FACTS Applications must be completed by December 31st for review
